The person is referred to the psychiatrist during medical. The psychiatrist evaluates to see if the person is fit to be inducted in army or not.

You will be referred to the surgeon during medical to get fitness for the surgery. If he marks you fit, then you can join.

If a person harm themselvess with the blade and make a cut on the arm at the age of 17 due to depression and how they are 22-23 years old ? Then they are fit or unfit for pakarmy ?
